How do I set up controlled vocabulary searching? For example, a search for heart attack would bring up results for myocardial infarction.

In Config->Institution->Discovery->Other->Customer Settings, change the use_controlled_vocabulary_search_expansion value to true.
Make sure that it saves and that you see an update date next to it when you're done.
